在 read、read_image 与 vision_* 打开文件之前先处理：长边超 2048 像素或体积超 8 MB 的图降采样（同一轮可降到 1280 再试一次），BMP/TIFF 转 PNG，按字节纠正标错的上传类型，并把 docx/xlsx/pptx、有文字层的 PDF、csv/tsv 与 html 转成 Markdown，不依赖任何外部程序。
